Appointment Policy
We are thankful to have all of you as guests in our office!
To ensure everyone receives the utmost care and attention, we ask that everyone arrives on time for their appointment. If you do not arrive at your appointment within 15 minutes of the scheduled time or cancel with at least 24 hours’ notice, you will be charged a cancellation fee to your ledger.
We ask that you fill out your paperwork ahead of time. Please allow an additional 10 minutes before your appointment slot to fill out paperwork if you are unable to do so online via the link texted to you.
If you are using any insurance, it must be provided and confirmed by our staff ahead of time to ensure you are eligible and able to use your benefits the day of your appointment. Please make sure that staff is able to reach you about any insurance questions we may have, so we are able to have your appointment go smoothly on your visit!
The Optomap Retinal Exam is included in every Comprehensive Exam visit. If you are using insurance at the time of your visit, the copays for Comprehensive Exam and Retinal Imaging will be listed on your insurance benefits page. If you have any questions, feel free to reach out to our friendly staff for clarification!
We ask that a maximum of one person accompanies the patient throughout the exam process. This allows for adequate space to perform all exam functions.
If you are being fit in contact lenses, the Contact Lens fitting is above and beyond the Comprehensive Vision Exam. The fees for this part of the exam vary based on the complexity of the Contact Lens fitting process. For patients using vision insurance, the copays for this are included in your insurance benefits page as well.
To achieve the best vision possible, we ask that you invest in all contact lenses and glasses materials in-office. We cannot guarantee the accuracy or quality of outside products and materials. In our commitment to providing you with the best vision possible, we build our materials packages to complement and enhance the results of your vision exam.
